ObjectiveTo evaluate whether sildenafil (Viagra) is superior to tadalafil (Cialis) in the treatment of pulmonary arterial hypertension (PAH), in a group of patients who were admitted with acute severe PAH (Sildenafil + Tadalafil).
MethodsTwenty-eight patients were admitted with PAH with an estimated hemoglobin level of 12.0 g/dL and a mean pulmonary vascular resistance (PVR) of 4.2 mm Hg or more. The clinical assessment included hemodynamic parameters, echocardiography and chest x-ray findings.
ResultsSix patients were diagnosed with PAH who were treated with Sildenafil + Tadalafil and had a PVR of 4.2 mm Hg or more. Six of the eight patients had Sildenafil + Tadalafil + pulmonary hypertension (Sildenafil + PAH). Mean follow-up was 5.7 months (standard deviation = 1.7 months). The two Sildenafil + Tadalafil + PAH patients had a mean hemoglobin level of 12.0 g/dL and a mean pulmonary vascular resistance (PVR) of 4.2 mm Hg or more. One patient had a PVR of 4.1 mm Hg or more and the other patient had a hemoglobin level of 8.3 g/dL. The pulmonary hemodynamics and hemodynamic parameters of the two patients treated with Sildenafil + Tadalafil and a mean pulmonary vascular resistance (PVR) of 4.2 mm Hg or more showed statistically significant differences at 3 months (mean difference: -.9 mm Hg; PVR: -.7 mm Hg; Fig. ). There was also a trend of decreasing right ventricular systolic pressure, which was also a factor in the regression model.
ConclusionsSildenafil and tadalafil were more effective in the treatment of PAH than Sildenafil alone. These results suggest that Sildenafil + Tadalafil may be more effective than Sildenafil alone in patients with pulmonary hypertension.
